1. Multidisciplinary Care Approach
Given the multisystem involvement, patients benefit from coordinated care involving:
• Clinical genetics
• Pediatric neurology
• Gastroenterology/hepatology
• Developmental pediatrics
• Nutrition/dietetics
• Therapies (PT, OT, speech, behavioral)
2. Neurological and Developmental Surveillance
• Developmental assessments: Regular evaluations of motor, cognitive, and speech milestones.
• Neuroimaging: Brain MRI may be indicated to assess structural abnormalities.
• EEG: For patients with seizures or abnormal movements.
• Behavioral evaluations: Screen for autism spectrum disorder and other neurobehavioral concerns.
3. Hepatic Monitoring
• Serum bile acids: Elevated in most symptomatic individuals; useful for diagnosis and monitoring.
• Liver enzymes: ALT, AST, ALP may be mildly elevated; GGT and bilirubin typically normal.
• Abdominal ultrasound: Assess for hepatomegaly, gallstones, and liver echogenicity.
• Pruritus management: Ursodeoxycholic acid (ursodiol): May reduce bile acid levels and improve symptoms. Consider cholestyramine or rifampin if pruritus is refractory.
4. Nutritional and Gastrointestinal Support
• Growth monitoring: Track weight, height, and head circumference.
• Fat malabsorption: Assess for steatorrhea, chronic diarrhea, and vitamin deficiencies.
• Supplementation:
• Fat-soluble vitamins (A, D, E, K)
• Iron if anemia is present
• Feeding support: May require nasogastric or gastrostomy feeding in severe cases.
5. Genetic Counseling
• Inheritance: Autosomal recessive; high recurrence risk in consanguineous families.
• Carrier testing: Recommended for parents and at-risk relatives.
• Prenatal or preimplantation genetic diagnosis: May be considered in future pregnancies.
6. Longitudinal Surveillance
• Head circumference: Monitor for progressive microcephaly.
• Growth parameters: Evaluate for failure to thrive.
• Liver function: Periodic monitoring even in asymptomatic individuals.
• Neurodevelopmental trajectory: Adjust therapies and educational plans accordingly.
7. Emerging Considerations
• No targeted therapies currently exist for WDR83OSdeficiency.
